What tinnitus is, and why it is not "just ringing"
Tinnitus is sound you perceive without an external source — ringing, hissing, buzzing, roaring, cicadas, a pulse. It is a symptom rather than a disease, which is why "what causes tinnitus" has no single answer and why treatment starts with working out which mechanism is driving yours.
The most common version travels with hearing loss. When the cochlea stops delivering certain frequencies, the auditory system does not simply go quiet at those pitches — it turns up its own internal gain, and that amplified neural activity is perceived as sound. This is why tinnitus so often matches the exact frequency region where hearing has dropped, and why restoring real input to that region so often helps.

The honest summary: nothing reliably makes tinnitus disappear, several things reliably make it matter less, and the supplement aisle is not one of them.
Treating the hearing loss underneath
For tinnitus that travels with hearing loss, well-fitted hearing aids are the single most effective step. Restoring real sound to the quiet frequencies gives the system something other than its own noise.
Sound therapy and masking
Low-level background sound reduces the contrast between the tinnitus and silence. Most modern hearing aids have dedicated tinnitus programmes built in.
Habituation-based approaches
Structured programmes that retrain attention, so the signal stops being flagged as important. Slow, and genuinely effective for many people.
CBT for the distress
Cognitive behavioural therapy does not quiet tinnitus. It is the best-evidenced way to reduce the anxiety and sleep disruption that make it unbearable.
Protecting what you have
Further noise exposure tends to make tinnitus worse. Custom protection is cheap next to the alternative.
What we do at the appointment
We start with a full hearing evaluation, because the pattern of loss usually explains the tinnitus, and because a tinnitus assessment without an audiogram is guesswork. We look in the ears — impacted wax alone can trigger or worsen it. We ask about onset, whether it is one ear or both, whether it pulses, what medications you take, and what your noise history looks like.
From there we match the picture. If hearing loss is driving it, we will talk about amplification and demonstrate what masking sounds like rather than describing it. If the more pressing problem is that it is stopping you sleeping, we will say so and point you toward help for that, because a hearing aid is not a treatment for insomnia.
We will also tell you if we think we cannot help much. Some tinnitus does not respond well, and setting a realistic expectation is more useful than selling optimism.
When tinnitus needs a doctor, not us
Most tinnitus is not dangerous. A few presentations are, and they should not go through a hearing aid appointment first.
